How Long Will My Appointment Take?
Our physicians take pride in giving each patient the consideration of their full time and attention during their appointment.  Great care is taken with each patient’s treatment, from fully explaining diagnoses and care options, answering the questions patients have and developing the best treatment plan for the diagnoses and the patient’s circumstances.  We recommend that you allow two hours for your initial appointment.
What About Medications?

You may be given prescriptions and instructions for medication necessary for your treatment.  If you have a question about medication we have prescribed or need a refill, please call us during office hours and we will be happy to assist you.  Prescription refills and requests should be made 2 to 3 days before your last dose by calling our office, or calling your pharmacy. 

We will call you if the prescription cannot be refilled or if we have questions.  Contact your pharmacy 24 hours after leaving the refill request with our office.
What is Your Cancellation Policy?


We at The Spine Center want you to get the best results from your therapy and will work with you and your physician to achieve them.  For the best possible outcome, it is important that you attend all of your scheduled therapy sessions.  While we realize that emergency situations sometimes make it impossible to keep an appointment, cancellations will delay your return to optimal health and affect our ability to provide the most efficient care to all our patients.

  • If you must cancel, please call us at (517) 337-3080 or toll-free at 1-888-712-6999 at least 24 hours before your appointment.  If it's after business hours, please leave a message in our general delivery mailbox.
  • Patients who do not attend a scheduled therapy session, or who provide less than 24 hours notice of cancellation may be charged a $25 fee for each occurrence.  Patients with a repeated record of no-shows or cancellations will not be allowed to schedule appointments in advance, but may be allowed to schedule same-day appointments as they become available.  In some instances, a patient may be discharged from treatment if it appears the patient is unable or unwilling to regularly attend therapy sessions.
  • It is your responsibility to arrive for your appointments on time.  If you are more than 10 minutes late, you might not be treated that day.
  • It is your responsibility to dress appropriately for your therapy appointments.  Your therapist will let you know what you need to wear.  You may use our locker room facilities for changing clothes.  Lockers are available for your use.  If you wish to lock your personal belongings in a a locker, lease provide your own lock.  If you plan to change your clothes before your therapy session, please arrive early to allow time to do so.  Changing time is not part of the therapy appointment and can reduce your treatment time.
 

What If I Have a Medical Emergency?
If you have a medical emergency, please go to a hospital emergency room.  Be sure to give the name of your physician from our office, so that the hospital emergency physician can contact us about your condition.

How Do I Get My Medical Records?


The Spine Center requires written authorization to release medical records. Prepayment is required for all medical records as per the fee schedule below. Please allow 7 to 10 working days to process your request. However, please note that older records that have been placed in storage will be pulled only on the first and third Fridays of each month. Consequently, additional time may be required to process requests for older records

Fees for copying medical records:
• $20 initial fee per request
• $1 per page for the first 20 pages
• $0.50 per page for pages 21 through 50
• $0.20 per page for pages 51 and beyond


Fees for Worker’s Compensation records:
• $2.50 retrieval fee for each 15 minute period
• $0.45 per page
• Postage cost


Form Completion/Attending Physician Statement Policy:
Prepayment for form completion is required as per the fee schedule below. Please allow 7 to 10 working days to process your request.


Form completion for attending physician statements:
• The first form/physician statement is completed at no charge.
• Additional forms/physician statements are $10 each.
